New Discipline Referral Form: Must Be Returned to Teacher Within 5 Days

 

Through the Consultation process, district representatives and Alliance representatives agreed to revisions to the SAISD Discipline Referral Form. According to the SAISD Student Code of Conduct, a teacher is required to submit a discipline referral when the teacher believes a student has violated the Student Code of Conduct. The new form was to be presented to teachers by not later than the October 25 th staff development day so that implementation could begin on November 1. Key changes to the form include:

 

The addition of an asterisk to mark the fields teachers are required to complete

 

A requirement, stated on the form, that the form be returned to the referring teacher or staff member after administrative action and within 5 working days of the day of submission.

 

The notation “if applicable” on the “Previous Action Taken by Teacher/Staff” section so that it is clear that the actions listed are not required prior to a teacher referring a student.

 

All discipline referral forms will now be tracked including those that are not considered a PEIMS offense. The new form is posted on the district intranet under the Administrative Procedures section.
